What is Botox/Dysport/Xeomin?

Botulinum Toxin type A was first explored for medical uses in the 1950’s. Botulinum toxin received its first FDA approval as Botox in 1989 for medical purposes, and in 2002 it received FDA approval as Botox Cosmetic for cosmetic treatments of facial lines and wrinkles.  Learn more.

Where is Botox/Dysport/Xeomin used?

These non-surgical injectables are FDA-approved to treat the frown lines between the eyebrows (Glabella). They are also used off-label to treat other areas such as periorbital wrinkle (crow’s feet), brow asymmetry, lip wrinkles, and platysma stranding (neck banding). Learn more.

When will I see results?

Generally results are seen in 3-5 days, although it may take a full two weeks to see the results. Typically patients will see the effects of the Botox/Dysport/Xeomin for 3-4 months.

What can I expect during the treatment?

During the treatment, which generally only takes a few minutes. Several tiny amounts of Botox, Dysport, or Xeomin are almost painlessly injected directly into the muscle. No anesthesia is required, however if you are concerned, we may apply topical anesthetic or cold packs to reduce any discomfort.

What can I expect after my treatment?

Immediately after your treatment, most patients look the same. Results of the treatment start being seen in 3-5 days after the treatment. Some patients may have little red dots for a few hours after. Bruising is uncommon, however, it may occur and usually resolves within a week. Learn more.

What are the possible side effects?

Possible side effects of Botox, Dysport, and Xeomin include possible discomfort, bleeding and bruising at the site of injection. A very small percentage of cases there can be drooping eyelid or ptosis, which resolves with time and sometimes with treatment. Learn more.

Understanding the Aging Process

Aging is a part of life, and as the years pass, we start to lose the youthful vibrant look we had for so much of our lives. Having an understanding of this process is key to treating it.

The aging process has multiple aspects such as genetics, fat, skin, muscle and bone. When you look at a more youthful face, such as a child’s face, there is the right amount fat, skin, muscle and bone in all of the right places. As we age, fat is lost, gained, or redistributed, depending on the area. This is what leads to circles under the eyes, gauntness and a tired appearance. Skin undergoes several changes as we age, including becoming thinner, drier, and it loses elasticity and collagen, which all allow for the skin to wrinkle or sag. These skin changes can be accelerated by smoking and sun exposure. Lastly we lose bone as we age. Bone is what gives the overlying muscle, fat and skin support and helps to hold them all up. The results of all of these changes are facial lines, wrinkles, and skin laxity, which make the face appear to have aged.

What are fillers? What do fillers do?

Dermal fillers are injections that add volume and fullness to correct moderate to severe wrinkles and folds on the face, including the lines from your nose to the corners of your mouth (nasolabial folds or laugh lines), the corners of the mouth, the area that extends downward from the corners of the mouth to the jawline (marionette lines). Fillers also can be used to define and plump or define the lips. Dr. Manolakos uses these products to give you a natural more youthful appearance. There are several dermal fillers and each one has its own unique formulation.

Types of Fillers

There are several dermal fillers and each one has its own unique formulation. Here are the fillers we use at Gold Coast Dermatology Center:

Restylane, Perlane, Juvederm, Voluma, and Belotero are a clear smooth gel formulation of hyaluronic acid. Hyaluronic acid is a naturally occurring substance in your skin. These products are specially formulated to act like your body’s own naturally produced hyaluronic acid, helping to instantly, visibly reduce moderate to severe facial wrinkles and folds or add fullness to your lips.

Radisse is also a filler, however if is made of tiny calcium-based microspheres suspended in a water-based gel. The calcium microspheres are similar to minerals found naturally in the body and over time, the microspheres and gel are absorbed leaving only natural collagen.

Sculptra aesthetic is a different type of facial injectable made from poly-L-lactic acid, which helps stimulate collagen growth. The main ingredient is a biocompatible, biodegradable synthetic material that has been used by physicians for decades. This injectable is different than the others as it is a collagen stimulator and therefore needs an average of 3 sessions over a few months. See more details under Sculptra.

Which Filler is Right for Me?

Restylane, Juvederm, Voluma, Radisse, Sculptra – which filler is best for you? All of these products are very similar FDA approved products that have minor differences like size of particles, depth of injection, frequency of treatments. Choosing the right one for you is dependant on multiple factors, like what area is being treated, how soon do you need to see your results, what other treatments have you had. The best way to decide is by talking with Dr. Manolakos, or your doctor. Choosing a board certified dermatologist or plastic surgeon, means you have chosen a doctor who is trained to do these procedures.

How is Treatment Performed?

Dermal Fillers are minimally invasive; they are injected directly into the wrinkle or the lips in our office. Patients are pre-treated with a topical numbing cream to make the injection comfortable. In addition most of the products have numbing medicine in them to help with the discomfort. Most patients feel only minimal discomfort.

Normal Duration of Fillers

Dermal fillers fade very slowly and gradually, so for the vast majority of patients, results should last from 6 months to a year. After approximately 6 to 12 months, the effects of the dermal filler gradually weaken, until your skin returns to its natural state. The specific amount of time dermal fillers last varies depending on the patient’s skin, the treatment area, and the type of product used.

As treatment with dermal fillers is temporary, they require touch-up injections for maintenance, and eventually repeat injections. At Gold Coast Dermatology Center, Dr. Manolakos will determine the ideal time interval for your follow-up treatments, and develop the optimal injectable regimen to suit your needs. Alternatively, if you decide not to maintain your new appearance, the results will gradually disappear over time.

Possible Side Effects

In 2012, over 1.9 million injections with dermal fillers were performed. Most side effects of fillers are temporary such as bruising, swelling, slight discomfort (area may feel tender like a bruise), nodules or mild redness at the injection sites. As with all procedures that involve an injection through the skin, there is a risk of infection. There are some more serious risks that are very rare, such as scarring. Pregnant or breast-feeding woman should not have dermal fillers. People with previous bad allergies, particularly to gram-positive bacteria, or that required in-hospital treatment, should not use dermal fillers.

IPL Photorejuvenation (photofacial)

Sometimes called a laser, IPL (intense pulse light) is a light-based treatment very successful for helping with age spots, brown spots, redness, sun damage, red blood vessels, and improving skin tone and texture. Gentle and non-invasive, an IPL photo-facial can be used to treat any skin surface, but most commonly is used for the face, arms, hands, and neck. Positive results are evident after the first visit, and downtime is minimal.

What to expect?

  • Mild post-treatment stinging may occur and swelling and redness can last one or two days. Treated spots take up to a week to fall off.
  • One to three treatments are usually needed to achieve desired results. Maintenance treatments are encouraged once or twice a year for the best outcome.

Resurfx (resurfacing laser)

ResurFX is a newly FDA-approved laser that works to improve fine lines and wrinkles, soften scars, address freckles, improve skin texture and tone, and promote collagen growth with minimal downtime. Fractional non-ablative laser resurfacing provides treatment both on the surface and deep in the dermis with a single treatment, this results in collagen production and skin tightening.

What to expect?

  • ResurFX is completed as a series of treatments. Because it is a limited downtime procedure, it requires multiple treatments for significant and meaningful results.
  • Immediately following treatment, you will have some degree of redness and swelling, which usually fades over about a week. Your physician will give specific instructions about what to expect for your particular treatment and how to minimize side effects.


Photofractional is a state-of-the-art, non-ablative photo-fractional skin surfacing technology that is used in conjunction with the Intense Pulsed Light (IPL) system. By combining these two powerful tools, you and your clinician receive greater convenience and customization for different skin conditions such as age and sunspots, fine lines, acne scars, uneven texture and skin tone.

What to expect?
Immediately following treatment, you may experience some redness or swelling, depending on your customized treatment. Most patients are able to resume their daily activities the very same day. You should also stay out of direct sun for a few days following treatment and apply sunscreen.

Photodynamic Therapy (PDT)


Photodynamic therapy (PDT) is an FDA approved treatment used to treat precancerous lesions called Actinic Keratosis (AK’s) as well as acne, photoaging and even some superficial skin cancers. PDT is a unique treatment using a topical medication called Levulan (Aminolevulinic Acid, ALA). Levulan is applied to the skin then the patient is put in front of the light to activate the medication. The Levulin is activated by the lights and destroys targeted cells. This treatment often results in redness and scaliness, similar to having a sunburn for 1 day to over a week. Patients must avoid sunlight for 48 hours after the procedure.

Tiny, dilated blood vessels on the face near the surface of the skin are called telangiectasias. These veins are harmless and are commonly found on the chin, nose, and cheeks.
